30 - MAKING YOUR ANTIBIOTIC STEWARDSHIP PROGRAM MORE EFFECTIVE

A functioning Antibiotic Stewardship Program (ASP) has been required by regulation since November 2017. Many facilities are struggling to achieve meaningful results to show surveyors. This presentation will provide the essential roadmap.

32 - H.I.E. (HEALTH INFORMATION EXCHANGE) PANEL DISCUSSION

Arizona’s Health Information Exchange (HIE), HealthCurrent, is leading the way in providing access to critical health information by post-acute care providers. HealthCurrent has 100% of Arizona’s hospitals participating and has now embarked on a strong campaign to enlist post-acute care providers to participate.  Not only is there no cost to post-acute providers, if Medicare  and/or Medicaid funds are received, a $5000 grant for implementation may be available. The panel will discuss experience to date with the HIE, current and anticipated benefits as well as and understanding of the impact HealthCurrent will make long term for the benefit of those served by improved outcomes and satisfaction and reduced costs. This panel will be of interest to all levels of care as well as ancillary providers.

33 - A NEW SALES APPROACH IN SENIOR LIVING

This presentation covers the critical role of selling in the senior living industry. Utilizing atypical selling behaviors, the participant will learn to quickly develop relationships of trust with both internal and external customers. In addition, effective communication skills will be discussed which again lead to the formation of trust-based relationships. A recent study by ProMatura painted a bleak picture of selling in the senior living industry and our industry’s declining occupancy rates are evidence that we must radically change our approach. At its essence, the study proves that we have historically taken an inferior approach to selling. Using actual case studies, real world senior living-based stories and illustrative exercises, this presentation will introduce the participant to a completely new and superior approach to selling in our wonderful industry!

35 - TAKE CONTROL AND REDUCE AVOIDABLE READMISSIONS FROM YOUR SNF

The Skilled Nursing Facility (SNF) Value-Based Purchasing (VBP) program went into effect in October 2018 and placed the spotlight on reducing avoidable hospital readmissions. SNF readmission data was used to determine changes in Medicare reimbursement. This session will present Health Service’s Advisory Group’s Reducing Readmissions Preparation Program (RRPP), which is a self-paced program using Quality Assurance & Performance Improvement (QAPI) principles. HSAG will present statewide data as well as experiences from a 2018 RRPP pilot program. This interactive session will also provide attendees a chance to practice and discuss readmission strategies.
