Mirabella at ASU has developed a music-in-residence program bringing university students and residents together for an interactive musical experience. La Posada at Park Centre is a Continuing Care Retirement Community (CCRC) and transition of care happens regularly as residents move through the continuum of care. Moves often occur pre-planned in advance; however, that transition can happen unexpectedly after a hospital trip resulting in significant gaps in care with medication administration and gaps in services when resident’s expectations are not met.

Session Objectives:

  • Learn how communications and collaboration are improved between the inter- and intra- professional teams who residents are in contact with.
  • Learn how a dedicated position may reduce medication errors and delays in medication administration.
  • Learn to improve resident satisfaction in the transition through the continuum of care.
