REGISTRATION INSTRUCTIONS

PROVIDER MEMBERS AT SPECIAL GROUP RATE FOR GROUPS OF 3 OR MORE

• REPRESENTATIVE of COMMUNITY registers self, and submits number of registrants by April 14
• Each EMPLOYEE OF COMMUNITY registers self by April 14
• Arizona LeadingAge will send the Community Representative an invoice for all registrants at the special group pricing
• Full payment for the Group Registration invoice is due May 10
• Note: Registrations received after April 14 will be charged the Individual Registration Fees

TO REGISTER:

• Each registrant will need to register by completing an online registration form. 
• Be sure to review the conference schedule prior to registration.
• This will ensure that we provide the hotel with accurate attendee counts for meals, as to avoid wasted food and unnecessary expenses.
