Pre-requisite for the CNA/DCW-Caregiver Bridge program is to have a valid form of ID, and a valid CNA/DCW license in the state of Arizona, a valid fingerprint clearance card, and an Arizona food handler's card.

Bridge Training Program Structure

  • Application: Complete the event registration.
  • *Applications will be processed Monday - Friday 9 AM - 3 PM*

    • Step One: Submit the registration packet with additional documents.
    • Step Two: Attend, study, and complete the medication curriculum.
    • Step Three: Arizona LeadingAge will register the individual for the State Exam.
    • Step Four: Take the state exam and pass!

Assisted Living Manager Training Program

Pre-requisite for the Assisted Living Manager Training program is to have a valid form of ID, a copy of your Social Security card, an Arizona Caregiver Certificate obtained after August 2013 (unless you are licensed in Arizona as an MD, DO, RN, LPN, or Arizona Nursing Care Institution Administrator), a valid fingerprint clearance card, and a CPR/First Aid Card.

Arizona LeadingAge - Fall Prevention & Recovery 

  • The Arizona LeadingAge Fall Prevention & Training Program was built to provide comprehensive and ongoing education to aging services providers to comply with ARS 36-420.01.

    Each module presents a deep-dive into fall prevention, specific department approaches to fall prevention, and fall recovery through powerpoints, resources, and a quiz.

    After completion of the following modules with a quiz score at or greater than 75% with no more than two attempts an individual is eligible to attempt the final exam to receive a certificate.

    • Introduction to Fall Prevention
    •  Choose the module that relates to your aging services setting
      •  Skilled Nursing, Assisted Living Centers, Assisted Living Homes, or Adult Day Centers

RoP Interpretative Guidance Phases 2 & 3

On June 29, 2022, CMS released revised Long-Term Care surveyor guidance, including clarifications and technical corrections of Phase 2 guidance issued in 2017, new guidance for Phase 3 issued in 2019, and guidance on the new arbitration requirements which became effective September 2019. Surveyors will begin using this guidance to identify noncompliance on October 24, 2022. This four-part educational webinar will explain the updates and changes of the regulations and guidance. Attendees will be able to gain knowledge on developing actions plans for achieving compliance related to the updates and changes. Attendees will be able to ask questions to gain clarification from an expert in health law.
