• 01) GROUP RATE (PROVIDER MEMBER-ONLY)
  COMMUNITY REPRESENTATIVE REGISTRATION & PROVIDER MEMBERS ONLY WITH GROUP RATE. Registration Fees to be determined by total number of registrants from community; then invoice sent to Community Representative for payment.
